The TPMS has two ways of working out what wheels are fitted and where.
The overall limit is 4 wheels which is internally set, therefore regardless of whether the spare is fitted with the sensor or not, you can only see the four wheels on the car.
Whenever a wheel is changed, moved around etc. The most reliable way of resetting the system is the manual way.
That is what I described. Force the system into learn mode and then walk around the car triggering the sensors. Because you have walked in a set pattern there is no further learning period and everything is set. This is the way recommended by Suzuki and the one dealers should do as it means the car owner sees any issues cleared when the car is taken away.
The second is the Automatic Way, this is what Bill was trying although the Suzuki guide recommends the manual method.. The Auto method where you simply let the car learn the tyres and sensors it has fitted automatically. This can take a while to settle down and can be erratic.
As documented elsewhere the automatic method is:
1. Turn off the ignition for AT LEAST 20 MINUTES.
2. Drive ABOVE 16MPH for AT LEAST 10 MINUTES. The system will relearn all the tyre IDs and their location.
The problem with the auto method is that if the sequence is interrupted it has to start again. Suzuki say that that includes the 20 minute ignition off. So it seems to be an unreliable method, although I believe that if you just keep on driving it will sort itself out.
(PS. In answer to my own question, I now know that if you have manually erased the codes but then not completed the sequence it will still complain and will relearn what it has fitted.)

So I tried again this morning.
Started from scratch and moved all 5 wheels around the car.
Went for a drive. After about 10 minutes without dropping below 25mph the dash lit up with the "TPMS Service" warning and a couple of amber warning lights. Drove on for about 5 minutes to find somewhere to stop.
Stopped with engine and ignition off for 25 minutes.
Started up again and TPMS warning still on. Drove for another 10 minutes without stopping.
No difference, dashboard still lit up with amber warnings.
I guess I'm gonna have to make a trip to the dealer when they're open and have them reset the system.
Seems odd that some people can do it without any issue.
Getting punctures and subsequently changing wheels around on a 4x4 must be quite common. Why is it so difficult!

Bill, this is why I expressed doubt in my original post about the auto update and promoted the manual way. The auto way works for some and not others. I have ordered a little reset tool to have a play and produce a video. Once I know I have a tool that lets me perform the proper manual process I will then play with the auto process

I must have reset the TPMS 3 or 4 times now and it all seems to happen without any hassle - other than scrolling through the menu options on screen till you find the reset option. I normally check my tyre pressures (when cold), scroll through the menu options and head off down the local dual-carriageway at about 50mph and don't stop until system resets. I'd say the reset usually completes within 2 miles or maybe 3 miles at very most. I'm struggling to think if Bill is just missing a key action and what that might be? I can sense the frustration at what should be an easily completed activity.
