If it suddenly breaks, go back to the last thing that you did before it broke and start looking there

Torque them up with the suspension normally loaded, the inner steel bush is locked to the bracket, and for long life of the rubber, you want it relaxed with the car standing, not twisted up (which happens if you tighten it with the suspension hanging down.)

Yes, I did in the end, I read in the workshop manual (linked above) the procedure for removing and refiting the axle.
