I've just had the same issues and it was down to the cat breaking up and blocking the exhaust. The fault code was for the o2 sensor.

But was it running great or good/normal when cold as I would have thought in the event of the cat collapsing it would be choked from the get go.

I pulled off the cat and middle section of the exhaust today and the cat "looks" normal. I say looks, because I cant see if but there is no debris coming out. I also flushed it through with water and the water came out clean. And as the exhaust section is only about 18 months old I would not expect it to be knackered just yet.
I did however have a slightly sticking front brake caliper so that will not be helping  , so I will see how it goes.
